Oral Nutrition Supplements (ONS) Guide for Filipino Patients with Dysphagia

Ano ang Oral Nutrition Supplements (ONS)?
Ang Oral Nutrition Supplements (ONS)—kilala rin bilang “nutritional drinks” o “supplement beverages”—ay specially formulated na inumin na puno ng calorie, protein, bitamina, at minerals. Ginawa para sa mga taong hindi kayang kumain ng solid food dahil sa disfagia, kalusugan na problemang, o malalaking kailangan ng nutrients.
Ang isang typical ONS ay naglalaman:

Ang mga taong may disfagia ay nahaharap sa triple threat:

- Mababang nutritional intake — Ang regular soft foods ay hindi sapat sa calorie at protein

Ang ONS ay tumutulong sa:
- Plug nutritional gaps — Direktang calories, protein, vitamins

MNA-SF Screening: Kung Sino ang Kailangan ng ONS?
Hindi lahat ng dysphagia patients ay kailangan ng ONS. Kailangan muna ng nutritional screening gamit ang Mini Nutritional Assessment Short-Form (MNA-SF).
Ang MNA-SF ay 6 tanong lang:
- Food intake — Bumababa ba ang dami ng iyong pagkain?
- Weight loss — Bumaba ang iyong weight sa nakaraang 3 buwan?
- Mobility — Kaya ka bang lumakad?
- Psychological stress / illness — May health problem ka ba recently?
- Cognitive problems — Sharp pa ba ang iyong isip?
- BMI or calf circumference — Ang iyong katawan ay normal weight pa ba?
Scoring:
- 0–7 points = Malnutrition (kailangan ng intervention, ONS)
- 8–13 points = Malnutrition risk (kailangan ng monitoring + ONS)
- 14+ points = Normal nutrition (ONS optional, focus on texture-modified soft foods)
Ang MNA-SF ay 89% sensitive at 82% specific para sa malnutrition risk (Rubenstein et al., 2001).
Importante: Kung ang iyong EAT-10 score ay ≥3 (dysphagia risk), dapat may ENT/speech-language pathologist (SLP) evaluation bago mag-start ng ONS. Maraming SLP sa Metro Manila (UP-PGH, UST-VRP, Makati Medical) pero kulang sa provinces.
IDDSI-Compatible ONS: Alin ang Ligtas?
Hindi lahat ng ONS ay ligtas para sa lahat ng IDDSI levels. Kailangan mong i-match ang viscosity (kati-kati) ng ONS sa iyong assigned level.

Key point: Most commercial ONS are naturally Level 2–3 (mildly to moderately thick) dahil sa kanilang natural protein + carbohydrate content. Kung Level 0 o Level 1 ang kailangan mo, dapat:
- I-dilute ang ONS 1:1 or 1:2 with water
- Pumili ng “pre-thickened light” variants (kung available)
- Consult ang iyong SLP para sa safe thickening agent
⚠️ WARNING: Kung mag-add ng thickener powder sa ONS, check ang label. Ang almagate, ibuprofen, at macrogol ay incompatible sa xanthan gum-based thickeners (Freire et al., 2021). Mag-consult sa pharmacist kung may medication.

Paano Gumagamit ng ONS: Practical Tips
1. Timing at Frequency
- 3 × daily (ideal) = 1 bottle every meal, OR
- 2 × daily (minimum) = 1 bottle every other meal, OR

2. Temperature at Storage
- Room temperature — drink as-is from bottle
- Chilled — refrigerate after opening; drink within 24 hours (tropical heat = bacteria growth faster)
- Heated — warm sa warm water bago mag-blend sa thickener (don’t microwave sealed bottle—risk of explosion)
- Opened bottle — must finish within 24 hours kahit refrigerated; mainit + humid ang climate natin
3. Blending with Thickener
Kung kailangan mo ng thickener (IDDSI Level 3–4):
1. Pour 150 mL ng ONS sa cup
2. Add 1–2 tsp modified food starch (cornstarch brand)

Maraming patients ay “tired” ng lasa ng ONS after 2–3 weeks. Tips:

- Mix with other foods — blend ONS sa soft porridge, custard, pudding
- Add vanilla or cocoa powder — para sa variety (if allowed ng SLP)
- Serve cold vs warm — some patients prefer cold (refreshing), others prefer warm (feels like “real” food)
- Smaller sips — drink 50 mL every 30 min instead of 237 mL at once (easier on stomach)
Dehydration Prevention: Why It Matters sa Pilipinas
Ang dysphagia + mainit na klima = very high dehydration risk. Bakit?
- Restricted fluid intake — dysphagia patients ay takot uminom ng liquids dahil aspiration risk
- High sweating — tropical heat natin ay nagpapataas ng fluid loss
- Thickened fluids — mas hindi kain-kumain, so lower total fluid intake

🚨 Stop ONS and consult immediately if:
- Nausea o vomiting pagkatapos uminom ng ONS
- Diarrhea or constipation lasting >3 days
- Aspiration signs: cough/wet voice after drinking, fever
- Severe bloating or gas
- Allergic reaction: rash, swelling, itching
