Oral Nutrition Supplements (ONS) Guide for Filipino Patients with Dysphagia

  3. Mobility — Kaya ka bang lumakad?
  4. Psychological stress / illness — May health problem ka ba recently?
  5. Cognitive problems — Sharp pa ba ang iyong isip?
  6. BMI or calf circumference — Ang iyong katawan ay normal weight pa ba?

Scoring:

IDDSI Levels at ONS Compatibility:

IDDSI Level Viscosity Range Safe ONS Choices
Level 0 (Thin) <1 mL in syringe @ 10 sec Pure water, unsweetened juice, Ensure Clear (if available), plain milk
Level 1 (Slightly thick) 1–4 mL Thin ONS products (Sustagen Junior, diluted Ensure 50:50)
Level 2 (Mildly thick) 4–8 mL Standard ONS (Ensure, Pediasure, Flexi, Sustagen)—no thickener added
Level 3 (Moderately thick) 8+ mL Standard ONS + optional mild thickener (cornstarch or modified food starch)
Level 4 (Extremely thick) Sits as mound on fork ONS + heavy thickener powder, OR specialty pureed ONS products (not common in PH)

Key point: Most commercial ONS are naturally Level 2–3 (mildly to moderately thick) dahil sa kanilang natural protein + carbohydrate content.

Product Type Calorie Protein Price (Approx) SLP Notes
Ensure (Abbott) Standard formula 220 kcal 9g ₱35–45/bottle Most common; good variety of flavors
Sustagen Milk powder or RTD 200 kcal 8g ₱30–50 Available nationwide; RTD form convenient
Pediasure (Abbott) Child-focused formula 150 kcal 5g ₱40–50 Lower calorie/protein; good for smaller appetite
Flexi (local brand) Standard formula 180 kcal 7g ₱25–35 Budget-friendly; less common but growing
Nutren (Nestlé) High-protein formula 240 kcal 12g ₱50–65 Better para sa sarcopenia; more expensive
Boost (Nestlé) Standard formula 240 kcal 10g ₱45–60 Good protein; available in major chains
Pure-Nutrition Local brand 200 kcal 6g ₱20–30 Emerging brand; limited availability

Where to buy: Major drugstore chains (Mercury Drug, Watsons, South Star), grocery stores (SM, Puregold, Robinsons, Savemore), online (Lazada, Shopee, Grab Health).

PhilHealth coverage: Very limited.

1. Timing at Frequency

Halimbawa:

Breakfast 7am: Soft porridge + 1 bottle Ensure
Snack 10am: (none)
Lunch 12nn: Minced & moist meal (IDDSI Level 5) + 1 bottle Sustagen
Snack 3pm: 1 bottle Ensure or Boost
Dinner 6pm: Soft meal (IDDSI Level 6) + (optional)
Bedtime 8pm: Warm milk (plain or + powder thickener if needed)

2. Temperature at Storage

3. Blending with Thickener

Kung kailangan mo ng thickener (IDDSI Level 3–4):

1. Pour 150 mL ng ONS sa cup
2. Add 1–2 tsp modified food starch (cornstarch brand)
3. Stir well para maiwasan ang lumps
4. Wait 2–3 minutes para ma-set ang consistency
5. Test sa spoon—dapat hindi mag-drip quickly
6. I-check vs IDDSI standard fork/spoon tilt test

Tips:

Dehydration Prevention: Why It Matters sa Pilipinas

Ang dysphagia + mainit na klima = very high dehydration risk. Bakit?

  3. Thickened fluids — mas hindi kain-kumain, so lower total fluid intake
  4. Reduced kidney function — elderly patients ay may slower kidney function

Common Mistakes with ONS (Avoid These!)

Mistake Why Bad Fix
Only ONS, no real food Boring + expensive; patients stop drinking after 2 weeks Combine soft foods + ONS (70% soft food, 30% ONS)
No MNA-SF screening May magbigay ONS sa hindi kailangan; waste of money Screen lahat bago mag-prescribe
Wrong IDDSI level Aspiration risk if Level 4 patient drinking Level 0 ONS Match viscosity sa assigned IDDSI level
Don’t check thickener compatibility Thickener + ONS may separate = poor consistency Test sa spoon first before drinking
Forget to refrigerate opened bottle Bacteria growth → food poisoning → vomiting, diarrhea Finish opened ONS within 24 hours; refrigerate immediately
No monitoring ng appetite Patient loses appetite sa regular food → malnourished anyway If appetite ↓, ask: too much ONS? Bad taste? Need flavor change?
Assume PhilHealth covers ONS Out-of-pocket cost = family stops buying after 1 month Check PhilHealth eligibility first; budget accordingly
Don’t track dehydration Patient becomes dehydrated → weakness, UTI → hospitalization Weigh daily; check urine color; log fluid intake
