Ano ang Dysphagia Thickener at Bakit Kailangan Nito?
Ang dysphagia thickeners ay mga asukal na ginagawang tibay ang mga likido tulad ng tubig, kape, at juice—ginagawang mas ligtas para sa mga taong may problema sa paggalaw ng pagkain. Ang mga pasyente na may stroke, Parkinson’s, o dementia ay madalas na kailangan ng thickened liquids para maiwasan ang aspiration (ang likido na napupunta sa hangin instead ng tiyan).
Ang International Dysphagia Diet Standardisation Initiative (IDDSI) ay bumuo ng 8 levels ng drink thickness:
- Level 0: Thin (tubig basta)
- Level 1: Slightly thick
- Level 2: Mildly thick
- Level 3: Moderately thick / Liquidised
- Levels 4-7: Solid foods (pureed to regular)
Ang iyong dokter o speech-language pathologist (SLP) ay magtatell kung aling level ang kailangan ng pasyente. Ang thickener mo ay dapat na dumaan sa IDDSI Flow Test para sure na tama ang consistency.
Tatlong Pangunahing Uri ng Thickener
1. Modified Starch (Processed Cornstarch)
Ano ito? Ang corn starch na nabago ng chemistry para mag-thicken ng liquids nang mabilis.
Advantages:
- Pinakamurang option — around ₱200–400 per 500g container
- Available sa lahat ng supermarkets at pharmacies
- Madaling gamitin — mabilis mag-dissolve
- No aftertaste compared sa iba
Disadvantages:
- Instable sa init at humidity — biggest problem sa Pilipinas! Kapag mainit at basang hangin, absorb ng starch ang moisture at nagiging mas tibay or nagsasawa
- Affected ng saliva enzymes (ptyalin) — nag-break down ang thickness sa loob ng loob ng bibig, kaya hindi safe for some patients
- Dapat i-refrigerate pagkatapos mag-prepare, kundi nagbabago ang consistency
- Mas mabilis lumipas ang shelf-life sa tropical humidity
Best for: Short-term use, hindi sa mainit na lugar, patients na may good oral control.
2. Guar Gum & Xanthan Gum (Plant-Based Gums)
Ano ito? Natural na plant extracts na nag-thicken ng liquids without cooking. Xanthan gum ay galing sa bacteria fermentation; guar gum ay galing sa guar seeds.
Advantages (Especially Xanthan):
- Nag-stay consistent sa mouth — resistent sa saliva enzymes, kaya ang thickness ay nananatiling stable habang kumakain
- Stable sa init at humidity — perfect para sa Pilipinas! Gel-based thickeners ay reliable sa tropical climate
- Transparent/clear — patient can see kung ano ang iniinom (good para sa acceptance)
- Compatible sa mainit at malamig na drinks
- Lower dosage needed — 1-2g lang para sa 200ml, so mas sulit ang bawat container
- Longer shelf-life sa humid environments
Disadvantages:
- Mas mahal — ₱600–1,500 per 100-125g container (Nestlé ThickenUP Clear)
- May slight “slimy” texture na hindi gusto ng ilang pasyente
- Mas mahirap hanapin sa local pharmacies (pero available na sa Watsons + Lazada)
- Dapat na-whisk bago gamitin (kailangan ng effort para sa elderly caregivers)
Best for: Long-term use, tropical climates, patients na sensitive sa taste, high-mobility patients na kailangan ng stable thickness while moving around.
3. Mixed Thickeners (Hybrid Formulations)
Ano ito? Combination ng starch + gum + other stabilizers para makuha ang best of both worlds.
Advantages:
- Balanced cost (medium range — ₱400–800)
- Better stability than pure starch
- Clearer than pure gums
- Convenient pre-mixed formula
Disadvantages:
- May variable results depending sa brand
- Hindi as effective as pure xanthan para sa salivary amylase resistance
- Need to shake well bago gamitin

Ang isang sikat na pag-aaral (Robbins et al. 2008) ay nagpakita na ang thickened liquids ay nakakatulong prevent aspiration sa stroke patients. Pero — maraming patients ang hindi nagsusumigil ng tubig dahil sa “lasa” ng thickened liquids, kaya nagiging dehydrated at sira ang kidney function.

Step 1: Alamin kung Aling IDDSI Level Ang Kailangan
- Dapat na prescribed ng doctor/SLP, hindi sariling desisyon
- Common: Level 2–3 para sa stroke, Level 1–2 para sa elderly na may difficulty
- Ask: “Anong IDDSI level ang kailangan ng pasyente?”

Common Mistakes Filipino Families Make
❌ Mistake #1: Gumagamit ng Plain Cornstarch
Problem: Regular cornstarch (like Fiesta brand) ay hindi formulated for dysphagia. Inconsistent thickness, may lumps, not IDDSI tested.
Solution: Buy modified starch thickeners na IDDSI-tested (Thick & Easy, Thick-It, local brands na certified).
❌ Mistake #2: Hindi Ini-adjust para sa Temperature
Problem: Gumawa ng thickened juice sa umaga, ipon hanggang tanghali sa init, nagiging mas thin or nag-separate ang consistency.

❌ Mistake #3: Hindi Sinusundan ang Prescribed Level

Problem: Walang IDDSI standardization. Consistency ay variable. May risk ng aspiration pag hindi proper.

❌ Mistake #6: Hindi Nag-mix ng Maayos

- Para sa xanthan/gum: Whisk well para mag-disperse ng particles, let sit 5 minutes, whisk again
IDDSI Testing at Compliance — Bakit Important
Ang IDDSI Flow Test ay hindi lang procedure — ito ay safety standard. Philippine hospitals (PGH, Makati Medical, Philippine Heart Center) ay gumagamit ng IDDSI para ensure consistent care whether patient ay nasa hospital o bahay.
Red flags na hindi IDDSI compliant:
- Product name na “nectar-thick” or “honey-thick” sa label (Hindi standard!)
