This oat milk banana smoothie achieves IDDSI Level 1 (Slightly Thick) naturally — without commercial thickener — through the starch released from a ripe blended banana and the natural viscosity of oat milk. The result is a smooth, lightly viscous drink that flows more slowly than water but is not “thick” in the way of a full thickened beverage. IDDSI Level 1 is appropriate for patients who have been assessed as needing minimally thickened fluid — always follow the speech-language pathologist’s specific IDDSI prescription.
The smoothie is designed as a breakfast option: naturally sweet, gently energising, and easy to prepare in under 10 minutes.
Ingredients (1 serving)
Main:
- 150ml unsweetened oat milk (barista or standard; avoid “light” versions which are more watery)
- 1 small ripe banana (approximately 100g; very ripe, with brown spots, gives better natural thickening and sweetness)
- 1/2 teaspoon vanilla extract (optional)
- Pinch of cinnamon (optional)
Optional fortification for calorie density:
- 1 tablespoon smooth almond butter or peanut butter (100% nuts, no lumps — adds protein and 90 kcal)
- 1 tablespoon rolled oats, blended very fine (adds texture and approximately 40 kcal)
- 1 tablespoon honey or maple syrup (adds sweetness if banana alone is not sweet enough)
Method
- Peel the banana; if frozen banana is available (pre-sliced and frozen overnight), use frozen for a thicker result.
- Place the banana, oat milk, vanilla extract (if using), cinnamon (if using), and any optional fortifications into a blender.
- Blend on high speed for at least 30 seconds until completely smooth — no banana chunks or lumps remain.
- Pour through a fine strainer once if you want maximum smoothness (optional but recommended for strict Level 1).
- Perform the IDDSI syringe flow test (see below) to confirm Level 1 consistency.
- Serve immediately in a cup or glass with a wide straw (or small spoon). Do not allow to sit — the banana starch will continue to thicken as it sits.
Texture Test
IDDSI Level 1 (Slightly Thick) syringe flow test:
- Fill a 10ml syringe with the smoothie; hold upright for 10 seconds
- Release the plunger (do not push)
- Measure the volume that flows out in 10 seconds
- Level 1 = 1–9ml flows out in 10 seconds
- Level 0 (too thin) = 10ml flows in under 10 seconds (treat as thin liquid — check with SLP)
- Level 2 (too thick) = less than 1ml flows in 10 seconds (in this case, thin with a little more oat milk)
Adjustment:
- To thin: add 20–30ml extra oat milk and blend again; re-test
- To thicken: add half a teaspoon of commercial xanthan gum thickener and blend; re-test
Safety Notes
⚠️ IDDSI level confirmation: This smoothie should naturally achieve Level 1 with a very ripe banana. However, banana ripeness and oat milk brand affect the result. Always test before serving.
⚠️ Straws: Wide-bore smoothie straws are preferable to narrow coffee straws. For patients with significant oral motor weakness, a teaspoon or small cup may be safer than a straw — confirm with the SLP.
⚠️ Nut butter: If using nut butter, ensure it is completely smooth (no lumps). Run it through the blender with the smoothie; do not stir in separately as it may not incorporate evenly.
⚠️ Banana allergy: Banana allergy is uncommon but exists. For banana-free Level 1 smoothies, use 1/2 avocado (adds natural thickness) or 1 tablespoon of cooked rolled oats blended smooth.

Nutrition
Approximately 200 kcal (without fortification), 3–4g protein, 40g carbohydrate, 3g fat. With smooth peanut butter: approximately 290 kcal, 7g protein. Oat milk contributes beta-glucan (a soluble fibre associated with cholesterol reduction and cardiovascular health), and in fortified versions provides B vitamins and calcium. Banana provides potassium, vitamin B6, and natural sugars for immediate energy. This combination makes the smoothie a nutritionally decent breakfast option for patients who cannot manage solid food in the morning.
Variation: Mango Version
For patients who prefer a tropical flavour, substitute the banana with 80g frozen mango purée (ensure no fibres remain — strain after blending). Frozen mango purée is available at City’super and selected supermarkets in the frozen section. Mango achieves a slightly less thick result than banana — adjust with a small amount of commercial thickener if the IDDSI test shows Level 0.
