Thickened Watermelon Juice | IDDSI Level 1 Recipe

Watermelon juice is one of Hong Kong’s most beloved summer drinks — naturally sweet, brilliantly red, and intensely hydrating. In its unmodified form, fresh watermelon juice flows at the same rate as water, placing it squarely at IDDSI Level 0 (Thin). For individuals who require thickened thin liquids, this poses a significant aspiration risk. By adding a small measured quantity of commercial thickener, the juice can be brought to IDDSI Level 1 (Mildly Thick), maintaining its characteristic flavour and colour while providing a margin of safety for individuals with mild dysphagia.
Ingredients (1 serving)
- 300g fresh watermelon flesh (rind removed, seeds removed)
- 2–3g commercial thickener powder (adjusted per product instructions to achieve IDDSI Level 1; common options include Resource ThickenUp Clear or Nestlé Nutilis Clear)
- 50ml cooled boiled water (to adjust consistency if needed)
- 1 tsp fresh lime juice (optional, brightens flavour)
- 1 small pinch of salt (optional, supports electrolyte balance)
Method
- Cut watermelon flesh into small chunks and place in a blender. Blend on high speed for approximately 30 seconds until completely smooth.
- Pour the blended juice through a fine-mesh sieve into a clean jug, pressing gently to extract all liquid. Discard the fibrous pulp. The resulting juice should be smooth and particle-free.
- Measure 200ml of the strained juice into a measuring jug or large cup.
- Following the manufacturer’s instructions for your specific thickener, add the appropriate quantity (typically 2–3g per 200ml for Level 1). Whisk briskly with a fork or electric milk frother for 20 seconds.
- Allow to stand for 1 minute to allow the thickener to hydrate fully, then stir once more.
- If using lime juice and salt, stir in at this stage.
- Perform a texture test immediately (see below) before serving.
Texture Test
IDDSI Level 1 (Mildly Thick) confirmation: Using a standard 10ml IDDSI syringe, fill with the thickened juice and invert for 10 seconds. More than 8ml but less than 10ml should flow out. The liquid should flow freely but with slightly more resistance than plain water — it should not move like syrup or form a slow drip.
Visual test: Viewed from the side of the container, the juice should flow easily when poured, with only a faint coating on the glass — no visible gel structure or clumping should be present.
Safety Notes
⚠️ Unstiffened watermelon juice is IDDSI Level 0 — natural watermelon juice flows as rapidly as water. For anyone prescribed IDDSI Level 1 or above, omitting the thickening step creates a meaningful aspiration risk. Never serve unthickened juice to individuals on thickened liquid diets.
⚠️ Thickener brands vary significantly — dosage and viscosity profiles differ between products. Always follow the manufacturer’s instructions for the specific brand being used and confirm with a syringe test on first use.
⚠️ Temperature affects thickened consistency — serve immediately after preparation. Refrigeration or extended standing can alter the viscosity of starch-based thickeners. Retest before serving if the drink has been stored.
⚠️ All seeds must be removed — even small watermelon seed fragments constitute solid particles in a Level 1 liquid. Ensure thorough sieving and visually inspect the finished drink before serving.

- Fresh watermelon: Available at all major supermarkets and wet markets from April to September; choose a melon with a bright skin and a yellowed ground spot indicating ripeness; pre-cut watermelon from supermarket chilled sections is a convenient alternative, best used within 2 days of purchase
- Thickener powder: Available at pharmacies (Mannings, Watsons), medical supply shops, and Hong Kong Red Cross outlets; common options include ThickenUp Clear and Nutilis Clear (both starch-based, flavourless)

Approximately 60 kcal per serving (200ml), 15g carbohydrate, 170mg potassium, 12mg vitamin C. Watermelon is 92% water by weight, making it an exceptionally efficient hydration vehicle. It also provides potassium, vitamin C, and lycopene. For individuals who underdrink due to dysphagia-related fluid fatigue, thickened watermelon juice offers a palatable and culturally familiar alternative to thickened plain water, supporting both hydration and micronutrient intake.
