Why Measuring Accuracy Matters
Thickener dosing errors directly affect whether a drink reaches the IDDSI level prescribed by the speech and language therapist (SLT). With some xanthan gum thickeners, a 1–2 gram difference can shift the result by a full IDDSI level.
The most common error among Hong Kong home caregivers is using visual estimation or ordinary teaspoons. Teaspoon volumes vary significantly by brand, and the difference between a heaped and a level teaspoon can exceed 50% of the intended dose.
The Standard Preparation Sequence
SLTs typically prescribe thickener doses by weight (grams) rather than volume, as weight measurement is independent of how the scoop is held. The recommended preparation sequence is:
- Measure the drink volume (typically 100–200ml)
- Weigh the required amount of thickener using a digital scale (0.1g precision recommended)
- Add the thickener in stages while stirring continuously
- Wait 1–3 minutes for full dissolution and stabilisation
- Verify consistency using the IDDSI Syringe Flow Test

1. Digital Kitchen Scale (0.1g Precision)
Standard kitchen scales (1–5g precision) are too imprecise for thickener measurement. Use a jewellery-grade or precision kitchen scale accurate to 0.1g for small-dose measurements.

- Manual stirring rod: Suitable for most xanthan gum thickeners; stir clockwise for approximately 30–60 seconds
- Small electric milk frother: Speeds up dissolution but can introduce air bubbles in some thickeners — test with the specific brand first
- Avoid forks: Insufficient mixing surface; thickener tends to clump between tines

This document can be updated at each SLT review appointment and shared with other family members or a domestic helper who assists with meal preparation.
